About Bank and Property Ownership Changes
Whether the Ownership of a property changes, the management agent for the property changes, or a bank account changes, CMS staff assist in these processes to ensure the correct Owner is paid timely.
The Owner and/or Agent must send in their new banking information as soon as possible. The forms that are needed to update a bank account include:

- Standard Form 1199A (Direct Deposit)
- W-9 (Request for Taxpayer Identification Number and Certification)
- Owner/Agent Questionnaire
- When the Management Agent has changed, HUD-9839-A, B or C
- When the Ownership has changes, the HUD-Approved HAP Assumption
